National CPC+ and Ohio CPC Programs

The application period for the Comprehensive Primary Care Plus (CPC+) program is open now through Sept. 15.

CPC+ is a national advanced primary care medical home model that aims to strengthen primary care through a regionally-based multi-payer payment reform and care delivery transformation. Ohio is one of 14 regions selected by the Centers for Medicare & Medicaid Services (CMS) to participate.

Ohio has been involved with the original CPC program since 2012. Practices that are chosen by CMS to participate in CPC+ and that meet Ohio's program requirement will be invited to enroll in the Ohio CPC program.
